Children who develop asthma have lung function deficits as neonates, study suggests

ScienceDaily (Mar. 30, 2012) ? Children who develop asthma by age seven have deficits in lung function and increased bronchial responsiveness as neonates, a new study from researchers in Denmark suggests.

"Previous research on the relationship between neonatal lung function and the development of asthma has been conflicting," said lead author Hans Bisgaard, MD, DMSci, professor of pediatrics at the University of Copenhagen and head of the Danish Pediatric Asthma Centre. "Our study shows that children with asthma by age seven already had significant airflow deficits and increased bronchial responsiveness as neonates. Lung function deficits also progressed throughout childhood in our study, suggesting a potential opportunity for early intervention."

The findings were published online ahead of print publication in the American Thoracic Society's American Journal of Respiratory and Critical Care Medicine.

The prospective study enrolled a birth cohort of 411 at-risk children of asthmatic mothers. Spirometry was performed at one month in 403 (98 percent) children and again at age seven in 317 (77 percent).

Significant neonatal airflow deficits, as measured by forced expiratory flow at 50 percent% of vital capacity and forced expiratory volume after 0.5 seconds, were observed among the 14 percent of children who developed asthma by age seven. Bronchial responsiveness to methacholine, which provokes narrowing of the airways, was also significantly associated with the development of asthma. Neonatal airway reactivity was a stronger predictor of asthma than neonatal lung function.

"We found that approximately 40% of the airflow deficit that was associated with asthma in our study was present at birth, while 60% developed through early childhood along with the disease," noted Dr. Bisgaard. "This indicates that both prenatal and early childhood mechanisms are potential intervention targets for the prevention of asthma."

The study used a homogenous study sample, which might limit extrapolation of the results to other populations.

"It seems that lung function changes associated with asthma occur very early in life and maybe even before birth," concluded Dr. Bisgaard. "This may explain the lack of effect from early intervention with inhaled corticosteroids and should direct research into the pathogenesis and prevention of asthma towards the earliest phases of life."

Lenovo ThinkStation D20

The Lenovo ThinkStation D20 is one of those systems that you know you will need. Are you building components for weapons systems that directly related to national defense? Are you in the editing and mastering stage of a $250 million Hollywood movie? Do you need to produce blueprints for an eight-lane automobile bridge spanning a major river? If you answer yes to one of these, you are probably in the market for a workstation desktop like the D20. If your job is eliminating pimples from a photo of a Kardashian, this system is overkill.

Design and Features
The D20 is a full-sized professional grade workstation, and it probably won't win any beauty awards. It's a humungous black metal and plastic tower, about 17 by 8 by 24 inches (HWD), so you'll have to clear up a bunch of space either under or on top of your work surface for the system. The D20 has a two-inch tall handle on the top, which will help your IT move the system around if necessary. The front panel is perforated with a hexagonal pattern, in order to aid airflow through the system. The tower is totally air-cooled, which reduces complexity over a liquid-cooled system. What's notable is even though the system is air-cooled, it is very quiet during use. The system's fans will only spin up fast during the boot process and if the CPU or GPU is really taxed. You'd think that a dual-CPU system would be loud because of the extra cooling fans, but during most work sessions the system was as quiet as a run-of-the-mill business desktop PC.

The system's massive size has a purpose: It can accommodate plenty of upgrades. The system we reviewed came with a 450GB 15,000rpm SAS drive, plus you can add up to four more SAS or SATA hard drives. The system can connect to industry-standard FireWire 400 and eSATA external drives in case you're dealing with hand-transported data. The system has a 2GB Nvidia Quadro graphics card in it, but you can add one more, plus another lower-end graphics card for multi-monitor support (there are two PCIe x16 slots, but one is only wired for PCIe x4). There's also space for another optical drive, plus nine memory DIMM slots. The system we reviewed came with 12GB of memory, but the system can handle 48GB total. Essentially, this system is overbuilt for most people who aren't aerospace engineers, trying to map the human genome, or designing bridges for government contractors.

The system is ISV certified to work with a plethora of software packages, including packages like AutoCad and PRO/Engineer. These certifications (and the underlying hardware and drivers) are important, since the users of a system like this may be designing the leading edge of an airplane wing or a strut on a bridge being built over a canyon. If you get an error on an ultra-high end gaming system you might have a missed frame while shooting an enemy in a virtual world. If a pro workstation user has an undetected error on a project, it could lead to a bus crashing into a canyon. This is one of the many reasons why professional workstations are more expensive than high-end consumer desktops.

Performance
The ThinkStation D20 exudes high-end performance. Its two Xeon processors, 12GB of memory, SAS hard drive, and Quadro 4000 graphics card mean that it will take any task you ask of it, and finish it quickly. The "X" in the X5687 processor's model number means that is can dynamically overclock itself like other Intel processors. The D20 finishes the Handbrake video encode test in a quick 1:19 and the Photoshop CS5 test in 3:09. This is over a minute faster than the last two dual-CPU workstations we've tested, the Editors' Choice?winning Lenovo ThinkStation C20 ($4,618 direct, 4 stars) and Apple Mac Pro ($3,499 list, 3.5 stars). Both workstations use "e" class Xeons that don't have Turbo. The wins extend to Cinebench R11.5, a simulation of 3D rendering in software (which is multi-core friendly). The D20 has the highest 12.65 score, compared to the 9.53 for the C20 and 8.62 for the Mac Pro. Granted, the Mac Pro uses a slower Xeon processor and is almost two years old at this point, but the Mac Pro is still being sold in this exact configuration. Remember what I said about gaming PCs? Well, the EC-winning Falcon Northwest Mach V ($6,899 direct, 4.5 stars) is able to perform better than the workstations shown here, but then again none of its components are ISV certified. The Falcon is faster than the D20, but it's not that much faster. You can't discount the Falcon's consumer nature. Think of the Falcon as a speed demon that you bring out to play, and the Lenovo as a precision instrument that also happens to be fast.

The Lenovo ThinkStation D20 is an interesting beast. It's something you buy to fulfill a specific need, rather than a system you can choose among a lineup. The closest system to the D20 in scope that we've seen recently is the Apple Mac Pro. Both systems are dual CPU, with plenty of internal expansion room. The D20 is quite a bit faster, but that's no surprise as the D20 has a faster clocked processor with Turbo capabilities. While that Mac Pro is a good choice for a user in a Mac environment, if your business has standardized on Windows you really want to get a Windows workstation. For general workstation and high-end business tasks like photo and video editing, the current workstation Editors' Choice HP Z210 Small Form Factor Workstation ($2,173 direct, 4 stars) has a better price/performance ratio, but the Z210 is a single-CPU system and isn't expandable like the D20. The D20 is really made for the scientific/engineering/movie mastering crowds, so it's more of a case where you know that you need it. In that case, the D20 will serve you and your users well.

Overall Responsibilities

The Finance and Budget Manager provides authoritative leadership on all budget, expenditure monitoring, financial reporting, and contract matters to ensure the timely and effective implementation of Project?s activities in DRC. S/he performs budgeting, expenditure monitoring, pipeline analysis, and reporting duties in accordance with generally accepted accounting principles and grant requirements, and MSH policy. S/he serves as a financial management resource to programmatic and operational staff. The Manager monitors transactions and subcontractors through the effective use of the review and approval process to ensure propriety with USAID and MSH regulations, including the preparation and award of performance-based sub contracts. With the Project Director, s/he monitors compliance with all internal and donor financial and administrative requirements. The Manager liaises with the country operations team to ensure that the project?s finance and operational needs are being met.
Specific Responsibilities

Lead and oversee the annual budget preparation, including oversight of the costing of the work plan and its integration with the approved budget.
Serve as local office lead with MSH headquarters for preparation of budgets and budget amendments to be submitted to USAID. Submissions to the USAID Agreement Officer shall be reviewed by the MSH Contract Officer or Senior Contract Officer prior to submission.
Develop procedures and programs for monitoring and analyzing program budgets, which allow accurate projection of expenditures and comparisons of actual and budgeted spending.
Monitor adjustments to the annual budget when required.
Track/Monitor monthly expenditures against approved budget.
Liaise with MSH headquarters on budgetary matters.
Coordinate monthly requests for funds, based on budget and cash flow projections, to ensure the project has all necessary funds for operations.
Review local Purchase Orders and confer with Project Director for approval, prior to issuance of the purchase orders by the COMU Director for project procurement of commercial goods and services within agreed upon birr thresholds
Work with the Cambridge-based staff to ensure any cost share target for the program is met.
Negotiation of local consultant rates upon delegation from the US-based Senior Contract Officer.
In coordination with the US-based Contract Officer or Senior Contract Officer, assist in issuance and administration of local subawardee/grantee, including fixed obligation grants, if any, and applicable lower-tiered contract organizations in:
Negotiating/analyzing and developing and annual work plans and budgets
Monitoring their progress against the plans
Tracking expenses against their budgets
Reporting on lower tiered subawardee/grante
Review sub invoices and work with US Based Contract Officer for approval of invoices for payment
Hire, train and supervise local staff with program responsibilities in finance, local subaward/grants and applicable lower-tiered contract.
Ensures project staff are familiar and compliant with USAID and MSH policies and procedures.
Liaises with the country operations management unit (COMU) to ensure the project needs are being met in terms of accounting, contracts, human resources, IT, and operations.
Performs other duties as assigned.

Qualifications

Masters degree in public administration, business administration, management or related area.
Five to seven years of progressively responsible experience in international development organizations.
Extensive experience with USAID contracting.
Experience with malaria control projects preferred.
Excellent planning, management, and organizational skills.
Proven leadership and interpersonal skill and capacity in negotiation and conflict management.
Fluency in written and spoken English and French (FSN-3 level or higher) required. Willingness to locate in Kinshasa and to travel within DRC.

Background Information
MSH is seeking candidates for an upcoming USAID-funded malaria control project in the DRC. Key project results areas may include: distribution of insecticide-treated nets (ITNs); prevention of malaria in pregnancy; malaria case management; and health systems strengthening and integration. The project will expand interventions to additional health zones in the four USAID Target Provinces. This position is contingent upon project award.

WASHINGTON (AP) ? The Supreme Court signaled Wednesday that it could throw out other key parts of President Barack Obama's health care law if it first finds the individual insurance requirement unconstitutional.

On the third and last day of arguments, the justices appeared to accept the administration's argument that at least two important insurance changes are so closely tied to the insurance requirement that they could not survive without it.

Less clear was whether the court would conclude the entire law, with its hundreds of unrelated provisions, would have to be cast aside.

The justices also spent part of the day considering a challenge by 26 states to the expansion of the Medicaid program for low-income Americans, an important feature in the effort extending health insurance to an additional 30 million people.

The court's liberal justices made clear they will vote to uphold the Medicaid expansion, which would take in 15 million people with the federal government paying almost all the costs.

Justices Sonia Sotomayor, Elena Kagan, Ruth Bader Ginsburg and Stephen Breyer voiced strong disagreement with the states' contention that the expansion of the joint state-federal program is unconstitutionally coercive.

"Why is a big gift from the federal government a matter of coercion?" Kagan asked.

The day's earlier session was unusual in that it assumed an answer to the central question in the historic health care case: that the requirement that Americans carry health insurance or pay a penalty will be struck down. In fact, if they follow their normal practice, the justices have not even met yet to take a preliminary vote in the case.

In their questions, the liberal justices took issue with Paul Clement, the lawyer for the states seeking to have the Patient Protection and Affordable Care Act tossed out in its entirety.

"What's wrong with leaving this in the hands of those who should be fixing this?" asked Sotomayor, referring to Congress.

Chief Justice John Roberts also spoke about parts of the law that "have nothing to do with any of the things we are" talking about.

For example, Ginsburg observed that the act deals with issues such as black lung disease.

"Why make Congress redo those?" she asked. "There are many things" that have "nothing to do with affordable health care."

But Clement said the court would be leaving "a hollow shell" if it decided to excise the several key provisions. "The rest of the law cannot stand," he contended.

Roberts and Justice Anthony Kennedy also asked hard questions of Deputy Solicitor General Edwin Kneedler that indicated they are at least considering Clement's arguments. Kneedler said that the only other provisions the court should kill in the event the mandate is stricken are revisions that require insurers to cover people regardless of existing medical problems and limit how much companies can charge in premiums based on a person's age or health.

Justice Antonin Scalia suggested many members of Congress might not have voted for the bill without the central provisions, and he said the court should not go through each and every page to sort out what stays and what goes.

"What happened to the Eighth Amendment?" Scalia asked, referring to the Constitution's ban on cruel and unusual punishment. "You really expect us to go through 2,700 pages?"

Opponents of the law, including Susan Clark of Santa Monica, Calif., also stood outside the court. Clark, who was wearing a three-cornered colonial-style hat, carried a sign that read "Obamacare a disaster in every way!"

"Freedom, yes. Obamacare, no," other opponents chanted.

The first two days of fast-paced and extended arguments have shown that the conservative justices have serious questions about Congress' authority to require virtually every American to carry insurance or pay a penalty.

The outcome of the case will affect nearly all Americans and the ruling, expected in June, also could play a role in the presidential election campaign. Obama and congressional Democrats pushed for the law's passage two years ago, while Republicans, including all the GOP presidential candidates, are strongly opposed.

But the topic the justices took up Wednesday only comes into play if they first find that the insurance mandate violates the Constitution.
